Your Child’s Medical Home
A medical home is… simply good care for children and families. The idea, which started decades ago as a way of improving the care of children with chronic and complex medical problems, has recently spread to include all children and adults as well and has become part of efforts at reforming and improving healthcare in the United States. Specifically in pediatrics, a patient-centered medical home is accessible, continuous, comprehensive, family-centered, coordinated, compassionate and culturally-effective care.
What it really means is that our practice is the central resource for all of your child’s healthcare needs. You have one doctor and healthcare team that knows your child best and coordinates all of his or her care. This team functions in partnership with you, the parent or guardian, because you know your child the best and are the most important part of the healthcare team.
We are now officially recognized as a patient-centered medical home. That means that we demonstrate the level of care we provide and continue to enhance and improve our care.

We make sure we are available when you need us, with appointments at convenient times, a portal, and more.
We continuously monitor the quality of our care and your experience and work to improve both.

We track your child’s need for care and notifying you when your child is due. We also track your child’s long-term health risks and act to prevent disease before it even starts!

Your role in the medical home
You can make your child’s care better & appointments go smoother if you:
- Are prepared at well visits to tell us how your child is doing in school and at home (sleep, play, development, etc.)
- Send someone familiar with your child’s symptoms if he or she is sick
- Bring a list of your questions and concerns to your child’s appointment
- Be sure to get us a copy of your child’s medical records
